If you work at sea, your ENG1 medical is non‑negotiable. It’s the UK statutory medical certificate that proves you are fit to work on a seagoing vessel and is required for most professional roles at sea, including many merchant ships, superyachts and fishing vessels.

What is an ENG1 seafarer medical?


The ENG1 is the UK certificate of medical fitness for seafarers, issued after a formal medical examination based on international maritime standards (STCW, MLC and related IMO/ILO guidance).


You typically need an ENG1 if you:

  • Work on a UK‑flagged merchant ship
  • Hold, or are applying for, a UK Certificate of Competency (CoC)
  • Work on certain commercial fishing vessels
  • Work on many commercial yachts and superyachts
  • Are at sea long enough or far enough from shore that emergency medical care isn’t easily available

What happens during your ENG1 medical?


An ENG1 is a thorough, practical fitness assessment – not a quick box‑ticking exercise. We follow current MCA standards for seafarer medical certification.

Your appointment will typically include:

Medical history review


  • Previous illnesses, operations and current conditions
  • Mental health history
  • Any history of blackouts, heart problems, seizures, diabetes or serious injuries

Medication and lifestyle review


  • Regular medicines, inhalers, insulin, anticoagulants, etc.
  • Alcohol, smoking and recreational drug history

Physical examination


  • Height, weight and BMI
  • Blood pressure and pulse
  • Cardiovascular and respiratory examination
  • Abdomen, nervous system, mobility and musculoskeletal assessment
  • You will be asked to undress to your underwear for a full, but efficient, exam – this is standard for ENG1.

Vision testing


  • Distance vision
  • Near vision
  • Colour vision screening (and referral for CAD/D15 testing where required by MCA rules)

Hearing test


  • Functional assessment and, where indicated, audiometry

Urine test


  • Screening for diabetes and kidney or urinary problems

Additional tests (e.g. ECG, spirometry) may be done or recommended if your history or examination suggests they’re needed to safely issue a certificate.

What your ENG1 result means


At the end of the assessment, your doctor will classify you according to MCA criteria as:

  • Fit without restrictions – full ENG1 for all duties
  • Fit with restrictions – for example, limited to certain jobs, waters or voyage types
  • Temporarily unfit – you do not currently meet the standard, but there is a realistic chance of improvement
  • Permanently unfit – you do not meet the medical standard for at least five years

If you pass, your ENG1 certificate is normally issued on the day, valid for up to 2 years (or 1 year in some circumstances, such as under 18s or certain conditions).

If we cannot issue the certificate immediately because further information or tests are required, you may receive a temporarily unfit certificate while the case is reviewed with the MCA where necessary.

You will receive clear, straight‑talking feedback on:

  • Whether you are fit for duty
  • Any restrictions applied
  • Any medical issues we’ve identified
  • What needs to change if you want to re‑apply after a fail or restriction

How to prepare for your ENG1 medical


To avoid delays or problems, you should bring:

  • Valid photo ID – passport, driving licence, seaman’s card or national ID
  • Your current ENG1 or equivalent certificate (if you have one)
  • A list of all regular medications
  • Glasses or contact lenses, plus spares if you use them
  • Any recent hospital or specialist letters
  • The name and contact details of your NHS GP

If you have previously failed an ENG1, had restrictions, or walked out of an exam before it was finished, you must tell the doctor – hiding this can invalidate a new certificate. 

 

How long does an ENG1 medical take?


30–45 minutes for a straightforward ENG1. More complex medical histories may take longer, especially if we need to review external documents or discuss treatment options.

We aim to run on time and minimise repeat visits – but we won’t cut corners just to move quickly.

Who needs an ENG1 medical?

You need an ENG1 if you are working at sea on a UK‑registered merchant vessel, many commercial fishing vessels, or if you are applying for or holding a UK Certificate of Competency (CoC). If you are in a more limited coastal or non‑seagoing role, you may only need an ML5 – we can advise.

In most cases, an ENG1 is valid for up to 2 years. For seafarers under 18, or in certain medical situations, the certificate may be limited to 1 year. GOV.UK+2UK Ship Register+2

The maximum fee for an ENG1 is set by UK regulations and is currently £115, with additional charges only allowed for clearly agreed extra tests (for example, a step test or advanced investigations). GOV.UK

If you are employed in the UK, your employer often pays for the medical. Check our Fees page or contact us directly to confirm the price you will actually pay.

Plan for around 30–45 minutes for a straightforward case. Complex medical histories, or situations where we need to review external reports or discuss adjustments to your role, can take longer.

Bring:

If you don’t meet the standard, you will either be:

  • Marked temporarily unfit – usually because a problem might improve with treatment or more information, or

  • Marked permanently unfit – where the condition clearly does not meet MCA standards for at least 5 years. GOV.UK+1

You will receive an ENG3 form explaining the decision and your right to request an independent medical review through the MCA. We will talk you through what the result means in practical terms for your current or planned job. GOV.UK

Yes. If you are found permanently unfit or your certificate is restricted and you disagree with the decision, you can apply for an independent medical review via the MCA within one month. An independent referee will reassess your case using the same medical fitness standards (MSN 1886).

Yes – ENG1s can be issued for younger seafarers, but they are usually limited to 1 year and the examination still has to meet full MCA standards.

Yes. If we find anything that affects your general health – blood pressure, weight, suspected diabetes or other issues – we’ll tell you clearly and can arrange follow‑up care or more detailed health screening at The Lagom Clinic if you wish.
